Daily Duties at Corazon Home Care:

During this episode of employment I case managed approximately 15-18 patients. All Medicare patients. Some with wounds, some with DM, some with anticoagulation defect and with foley catheters. Teaching focused on infection control, safety, getting organized with their DM and documenting readings, getting patients new glucometers due to theirs being broken. Mostly just assisting patients in taking charge of their health; therefore, preventing hospitalization with education
