Daily Duties at Hebrew Home At Riverdale:
Provide in-person intake assessments and formulate initial care plan for clients and families in a timely manner; as needed include written assessment reports and/or care plan summaries/recommendations; monitor condition of client to evaluate potential treatment and interventions; provide crisis interventions on a 24-7 basis. Provide case management to include linkage and referrals both in-house, and outside of agency. Advocate on behalf of client as a means of gaining access to appropriate services. Assist with clinical diagnosis, and provide psycho-therapy. Interface and collaborate with clinical team and medical staff regarding assessments, intakes, and coordination of ongoing client care. Obtain appropriate case consultation as needed. Communicate timely updates to family members and supervisor in regard to client progress, and/or treatment updates. Monitor and maintain accurate documentation of client services; ensure compliance with agencies policies and policies related to HIPAA. Develop and maintain clinical and care management skills through seminars, workshops, or other forms of continuing education/training related to older adults. Attend and participate in program, Agency staff meetings, and related community agency meetings as required/requested.
