Daily Duties at Alden North Shore Rehabilitation And Nursing:
Every morning I meet with new admission patients to discuss patient history, spiritual needs and advance directives such as living will, power of attorney of healthcare/financial and DNR. I monitor all my 80 patients significant changes to address any concern patients might need. I do charting at least 14-15 patients a daily. I meet with all 80 patients and family in careplan meetings prior to discharge. I meet with therapy department manager & resident care coordinator nurse to discuss all 80 patients insurance coverages. I call all private insuranc patients besides medicare to communicate to insurance case manager to ensure the highest quality of coverage is being met and provided. I discharge 3-5 patients daily to another facility, home or family members home. I coordinate range from 25-30 patients discharge with transportation, order equipment or supplies, set up out patient therapy or home health services, medical record and physician medication prescription prior to release from the facility a week.
