Daily Duties at Tandem Health Care:

Responsibilities included but were not limited to: Supervision of case management staff. Served as liaison and coordinated services within and between the multidisciplinary team, patient/family, facility, and outside agencies. Served as the facility liaison between the patient/family and Hospice services. Maintained case records in accordance with program and division policies and procedures. Conducted client screening and intake processes. Developed and implemented an appropriate case plan for clients, including measurable goals to be achieved, in accordance with program and division policies and procedures. Initiated and participated in team staffing and case conferences. Prepared and submitted reports and data as required. Managed psychotropic medication reduction. Facilitated staff trainings, including advanced directives and sensitivity training. Responsible for creating policies, procedures and implementing necessary programmatic changes for all case management staff to maintain program consistency. Participated in all quality improvement and technical assistance. Provided quality assurance reviews for all closed cases. Provided community outreach and education.
