Daily Duties at Family Health Network:

Develop and implement a Diabetes Case Management Program for Managed Care patients, pediatric and geriatric covered by Illinois Medicaid; in collaboration with primary care physicians. Goals of the program included, but were not limited to: Ongoing measurements and analysis of A1Cs and Lipids, with expected decrease in values that are above normal.
