Daily Duties at St. Jospeh Mercy Oakland:
• Precept pharmacy students and residents in the department of pharmacy practice • Review patient medication orders and profiles for allergies, interactions, therapeutic duplication and appropriateness of use • Communicate with healthcare providers regarding patient care •Prepare and dispense medications and IV admixtures for inpatient care •Participate in interdisciplinary group conferences across hospital/health-system to enhance patient care. •Provide pharmaceutical care by attending rounds and assisting medical staff in all facets of care related to medication therapy •Evaluate medication safety events and develop action plans to address safety concerns relating to the medication use process •Prepare medication error data analysis for presentation at monthly quality meetings •Involvement in various interdisciplinary committees focusing on enhancing and improving patient safety and care •Review and development of pertinent hospital, nursing and pharmacy policies and procedures •Respond to pharmacy practice issues submitted by staff, management or other multidisciplinary team members •Evaluating departmental practices for improvement in efficiency and process flow. •Implementation of departmental process changes as required. •Staff training as required with new procedures, operational systems, work flow processes, etc.
