Responsibilities: General Clinical Pharmacist duties including drug utilization review, medication order entry, dosing of aminoglycosides, vancomycin, heparin, low molecular weight heparin, coumadin. Close work with pharmacy technicians, nurses, physicians and patients. Various involvements with IV admixture both mixing and checking of technician. Total Parental Nutrition order entry via Abacus TPN. Patient discharge counseling. Specific Ambulatory Clinical Pharmacist duties including Anti-coagulation Bridge Therapy via collaborative pharmacy protocol for ambulatory patients including pre/post procedure and new coumadin management. Use of finger-stick INR. Collaborative care and discussion of PT/INR with patients, physicians and nurses. Patient counseling and discharge. Oversight, preparation, and monitoring of monthly infusions of various disease states including use of Remicade, Solu-medrol, Xolair, Prolastin, Orencia, Intravenous Immune Globulin, Tysabri, various antibiotics.
