Daily Duties at Southerncare Inc:

Provides skilled and compassionate nursing care to patients and families in their place of residence and coordinates activities of the Interdisciplinary Group (IDG) as directed on the patient Plan of Care. Performs ongoing comprehensive review of medical record documentation to ensure compliance with: State/Federal Rules and Regulations SouthernCare Policies and Procedures Discipline-Specific Standards of Practice Collaborates with the Clinical Director to facilitate a corrective action plan as related to State/Federal surveys and corporate compliance audits. Provides the Clinical Director with feedback on medical review audits and suggestions for educational in-services to improve documentation. Serves as a resource for patient admission and discharge appropriateness. Participates in on-call rotation as directed by the Clinical Director. Participates in IDG meetings. Complies with accepted ethical conduct and professional standards of Nursing Practice. Demonstrates exceptional service to patients, families, referral sources, co-workers and customers. Other duties as designated by the Clinical Director.
