Daily Duties at Oakland Grove:
Narcotic accountability, mentor new nurses, pass meds, document, assessment of skin and pain. Documentation requirements include, but are not limited to, the following: a. Changes in medical and physical condition; notify MD and family; b. Response to antibiotic therapy; and document c. Report of weight loss or gain and report to RD, MD anf family and begin a root cause analysis d. Report of peg/G tube patency as directed;and document e. Complete, accurately, and timely physicians' orders f. Changes in specialized diets; and patient's response to it as directed by SLP. g. Report of pressure ulcer size and depth, and periwound condition, current treatment, and any suggestion for continued use of present treatment or a change in treatment with supporting data of treatment weekly. Shift report to the certified nursing assistants must include, but are not limited to, the following: a. Changes in resident condition; b. Report of new admissions; c. Residents scheduled for appointments and rehabilitation; d. Assigned baths and restorative instructions; e. Splint applications; f. Changes in specialized diets; g. Individualized patient instructions; and h. Special attention needs to be given to the CNA's who are new to their assignments. Charge nurses are responsible for the knowledge of patient information through the maintenance of accurate data. This knowledge includes, but is not limited to (data must be shift specific): a. Resident skin condition and report changes to nurse b. Diet and supplements documented c. Special equipment needs and how tolerated d. Continentcy and type of protection used if incontinent; e. Weight gain or loss reported to nurse f. Side rail use and reason for use; (never as a restraint), As an enabler, and to assisit with (I) bed mobility and repositioning. g. Assistance required by resident; How many people and/or device used. h. Residents at high risk for skin breakdown requiring more frequent positioning and reporting to Nurse. i. inform CNAs of any patient on precautions and what PPE they will need. Licencesed Nurse Assessment of the residents' skin for breakdown done on admission, weekly x4 then quarterly Braden Scale (or Norton) analysis by personal physical assessment done of the resident by the charge nurse at bath time weekly.
