Daily Duties at Advocate Trinity Hospital:

I coordinate the Outpatient Diabetes Wellness Center. Our center is accredited through the American Diabetes Association under an umbrella with two other facilities. I maintain the quality assurance project while conducting outpatient diabetes education and training. I also see inpatients as a consult and have taken on the task of educating the entire nursing staff on diabetes "basics" and on glucose monitoring so they can provide this type of information to patients and families upon discharge, and by giving the client a glucose meter with education, we have helped decrease the frequency of re-admissions. I am marketing our services to physicians and identified that none of our providers are doing professional continuous glucose monitoring. There is a strong need for this in the service area we have and I convinced the hospital to purchase 2 CGM (continuous glucose monitors) so that we can have patients utilize this at home for a fee and not only make money for the hospital but also arrive at better and more cost efficient medication therapies for patients. Implementation of the Chronic Disease Center at Advocate Trinity by seeing diabetes and congestive heart patients on an ongoing and as needed basis to prevent hospital emergency room visits and hospital re-admissions. Responsible with inpatients and outpatients for: Assessing and advising diabetes/CHF patients on medication administration, home glucose and blood pressure monitoring and recording of the same, diet and exercise and long term complications of said disease. Develop an Inpatient hospital diabetes education and discharge program. Collect identified measures for ADA Recognition Program Certification. Collaboration with hospital physicians for patient referrals and feedback. Identify, plan and implement diabetes education for the hospital staff. Develop community based programs on diabetes
