•Developed and implemented the person-centered plan (PCP) for intellectually disabled (dually-diagnosed) individuals through team meetings and ensured the service delivery process and intake of 30+ comprehensive assessments •Established and maintained professional relationships with the individuals and their families, staff and community agencies through oversight of the individual’s related activities and provision of services •Maintained comprehensive clinical records to include documentation verifying service delivery processes such as the PCP, coordination of care and discharge planning •Implemented the Plan of Care (POC) with team members, habilitative instructors, families and individuals by determining task analysis’ for goals established
