Daily Duties at Southwest Network, Formerly Magellan Health Services:

•Served as a point of contact and collaborated with clinical team to engage, educate and coordinate care with 280+ adult service recipients whom are dually-diagnosed with a serious mental illness (SMI) and substance abuse addiction, their families and behavioral health through the implementation of services •Ensured the provision of all covered services identified in the individualized service plan (ISP) through coordination, delivery, and continuity of care between multiple service providers and supports through assertive communication •Built innovations and pursued best practice outcomes for persons with a mental illness including continued education, employment, substance abuse, independent housing and community tenure •Implemented behavioral health services, screened/assessed financial entitlements and completed 35+ assessments and service plans yearly in accordance with specified goals
