Daily Duties at Akron General Medical Center:
Planned for the appropriate discharge of patients to the next logical level of care beginning at time of admission. Ordered appropriate DME's, Medications, Home Care or other living arrangements, depending on case. Only Discharge planner in hospital on weekends so had to be familiar with all insurance, medicare, medicaid regulations in order to facilitate appropriate discharge. Care Coordinator of Care Management department once developed. Point person for all care managers for questions regarding discharge and assistance with submission of information to insuance claims. Wrote letters of appeal when payment was denied, involving chart review and knowledge of appropriate medical interventions.Recovered over 800,000.00 in first three months of program
