Daily Duties at Crossroads Hospice Neo:
Quality Improvement, data collection of internal and external surveys to assess problem prone areas, using Excel progrm to track information. Analysis of information collected to develop performance improvement solutions. Analysis of Key Production Indicators quarterly for purpose of quality analysis. Reviewed recertifications for timely submission and compliance, reviewed admission charts for appropriate documentation, chart audits for supporting hospice documentation and compliance with CHAPS, participated and led IDG team meeting every two weeks, followed up on problem prone issues for resolution. Triage of phone calls from staff, patients and families with follow up. Supervision of staff in field. Education of staff on documentation for hospice and developement of organizational tool to use in field.
