1. Ensured optimal medical nutrition therapy for 125 end-stage renal disease patients 2. Contributed as part of a multidisciplinary care team 3. Reviewed monthly lab work and assess nutritional status regularly 4. Responsible for bone and anemia management 5. Collaborated with doctors on daily patient rounds, medication changes, and plan of care 6. Completed appropriate short-term and long-term care plans for all patients in my care 7. Developed multiple nutrition handouts, activities, newsletters, and bulletin boards on a monthly basis 8. Strived to achieve quality indicators on monthly basis
