*Oversaw daily operations of the anticoagulation clinic, providing pharmaceutical care and collaborative drug therapy management to patients. *Collaborated with staff to develop work schedules and assignments in order to meet productivity goals. *Conducted annual performance appraisals of clinic staff; recommended development goals. *Laisoned between the anticoagulation clinic and other hospital departments; communicated with the ACC program manager regularly to keep manager up-to-date about clinic, hospital, and personnel issues. *Coached and led staff through ACC program changes; continually maintained a state of JCAHO readiness. *Provided collaborative drug therapy management including: initiation of warfarin, with or without LMWH/Arixtra ; warfarin dosage adjustment; protime monitoring using the Coaguchek XS protime monitor; patient and family education ; drug/food interaction monitoring; follow-up instructions. *Coordinated annual competency for the point of care testing monitor; collected data for quality assessment and improvement activities for clinic service. *Participated in the educational activities and programs for the hospital’s staff, and trained pharmacy students and residents in the area of anticoagulation.
