Daily Duties at Trinity Health Center And Rehab:

Started off on a LTC/Subacute unit.In am Report received from previous shift nurse, then pass along pertinent information to staff, then supervise them. Narcotic accountability, pass meds & narcotics, wound care, call physicians, as needed. Write nurses' notes, write and/or update careplans, enter new doctors into Sigmacare EMR system. Check patients for weight gain/loss, assess skin integrity and turgor, assess for signs and symptoms of pain and/or discomfort, assess change in ADLs or bed mobility. Follow PT/OT/SLP plan of care. Keep families informed of any changes in condition or incidents. All performed under the guise and ospice of The Nurse Practice Act, and Medicare/Medicaid guidelines (CMS). IV Insertion, PICC and Portacath Extended (Access) Care, PD, TPN, Fistula AND/OR Permacath Care,Geriatrics Hoyer, Form Designs, VS, Subacute Charting, SNF Guidelines, Assignment Sheets, MANAGED CARE, G/J TUBES, DRAINS (JP), AND MORE.
