Daily Duties at St. Michael’s Nursing Home:
• Maintains a working knowledge of applicable accreditation standards. • Proposes changes to and maintains in-house data base configuration. • Identifies and analyzes current client medical record management (UM, CM, DM) processes. Provides detailed documentation concerning meeting the patients’ needs. • Engages in advance preparation, with the interdisciplinary team, to implement plan of care through written and verbal consultation; • Determines the interaction between various health care departments and the ability of the team to function as envisioned by the doctor and patient. • Guides the team in workflow modification via electronic messaging, and telephonic trouble shooting. • Strong ability to effectively manage multiple office/clinical tasks, and coordinate time effectively to maintain documentation accuracy and productivity. • Thoroughly documents progress and process inconsistencies/ issues/ software concerns for follow-up and/or resolution by programming and field staff. • Coordinates data analysis needs for implementation and report specifications and recommend appropriate system configuration. • Perform material data entry assessments as a field nurse case manager
