AK Home care Services Director of Nursing/Case Manager, 12/2009-08/2010 • Assist company to pass Chaps certification Responsibilities: • Guided directed and planned the organization and coordination of agency activities in the areas of responsibility, with ongoing assessment of quality and efficiency of these activities. • Maintained the schedule, communicating with agency health care providers and managing all schedule requests for all nursing staff in regards to admissions, discharges, and recertification outcomes in a timely manner. • Investigated and resolved any grievances related to patient care. • Assessed staff on quarterly and annual basis to maintain high quality nursing care. • Assessed the need for and assisted in implementation of staff development programs to enhance professional growth of personnel. • Assumed all administrative responsibilities of the agency where applicable. • Development of the QA/QI program and reports. • Chaired Safety Clinical and Patient Safety Program and Meetings • Risk Management Designee • Development of Case Management and Discharge Planning Program • Development of health fairs for marketing, including recruitment of nursing staff
