Primary Home Health Care Director of Nursing/Case Management, 12/2009-09/2010 • Responsibilities included Management of home health staff to include RNs, LPNs, Home Health Aids and therapy dept., Utilization Review, Quality Assurance chart auditing • Coordination of care with home bound patients Medicare/ Managed Care patients • Staff education, orientation of new staff • QA/ PI projects to improve best practice and staff outcomes. Coordination of care with home bound patients. • Prepared staff, and charts for Jacho Survey • Maintained operating budget • Maintain Infection Control Program, PPD and Employee Health Records • Perform Supervisory Visits on a regular basis, performed patient satisfaction reports • ICD-9 coding of assessments, procedures, and medical diagnosis • Assist with scheduling of referrals, and performed health fairs. • Marketing of home health services to physicians, Hospital Case Managers, Discharge Planners and Social Workers
