Daily Duties at Star Home Care Services:

Star Homecare Services, QA Review/ Clinical Supervisor 04/2010-6/2011 • Care Management, Utilization Management, Chart QA auditing, and Coordination of care for 135 June census report for home health agency. • Supervision and direction of the Care Management Team and all aspects of care coordination for the medically complex cases to execute the patient’s plan of care for IV Therapy, Vents and Respiratory Equipment, Wound Vacs etc. utilizing extensive knowledge of local, regional and national resources and care coordination with social services, physicians, and therapy. • Mobilizes resources and interviews, as needed, to achieve expected goal to assist in achieving desired clinical outcomes within the desired time frame • Fiscal management utilizing extensive knowledge of Third Party payers, Medicare, Medicare Replacement, Medicaid and Commercial insurers including negotiation of rates for non-contracted payers. • Preceptor for newly hired case management nurses and nurse assistants. HOME CARE SERVICES RN CASE COORDINATOR • Initiates, schedules/supervises, and coordinate care of assigned patients . • Manages episode of assigned patients as evidenced by favorable patient outcomes. • Ongoing evaluation and education of client in holistic manner. • Supervision of LPN's and Home Health Aides. • Provides accurate, current documentation according to agency /regulatory policy and procedures within 24 hrs. • Plans visits to facilitate patient needs and effective episode management in compliance with regulatory agencies. • Maintains proficiency in clinical assessment skills. • Establishes and updates Nursing Care Plan every 60 days. • Participating and completion of monthly and admission case conferences. • Develops Plan of Treatment to reflect current needs at time of recertification to include all disciplines. • Assures physician orders for care meet regulatory requirements. • Maintains communication with physician and interdisciplinary team when additional patient needs identified. • Meets productivity target consistently. • Demonstrates good time management skills.
