Daily Duties at Tlc Home Healthcare, Inc.:

T.L.C. Home Healthcare, Inc. Director of Clinical Services • Supervision and provide direction of the Care Management Team with all aspects of care coordination, Care Management, Utilization Management, Chart QA auditing, and Coordination of care to patients and their family. • Strong ability to effectively manage multiple office/clinical tasks, and coordinate time effectively to maintain documentation accuracy and productivity. • Provided the ability to communicate effectively in order to obtain optional patient outcomes in addition to critical thinking with evidence-based practice. • Provide strong leadership, with interpersonal skills in communicating with peers and physicians. • Organizational skills to ensure compliance and patient confidentiality. • Maintained proficiency in all documentation and record maintenance to ensure accuracy. * Develops, evaluates, and revises the plan of care based on patient assessments and multi-disciplinary input. Develops recommendations for quality remediation plans Maintain extensive knowledge of regulatory and applicable accrediting body requirements becoming a Compliance resource for the team. Proficient in interpreting benefits, contracts, and medical policy and coding Maintain knowledge of State rules and Federal legislation/regulations as well as Community & State guidelines Establish and maintain good working relationships throughout the company to support goals and objectives. Demonstrate flexibility, availability, and dependability to conform to position responsibilities. • DIRECTS, COORDINATES AND PARTICIPATES IN THE ACTIVITIES REQUIRED FOR DELIVERY OF QUALITY PATIENT CARE AS PART OF THE HEALTHCARE TEAM. Directs and coordinates nursing care, establishes priorities and delegates nursing activities according to patient needs and staff's capabilities and qualifications. Evaluates and supervises nursing care and provides constructive and relevant coaching and feedback
