Daily Duties at Otterbein Portage Valley :

Sub Acute Unit RN/Supervisor for 12 bed Sub Acute unit providing total care to patients, along with Supervisor of total facility including but not limited to Long Term, Assisted Living and responsible for responding to Emergency in Apartments and Independent living Condominiums. All staffing issues conflicts responded with ease and promptness. Case management of skilled patients and hospice patients, on call duties, team leader. Responsible for primary care/case management of patients. IV’s, TPN, G-tubes, etc. Develop patient care plans that best fit the needs of the patient and maintain HIPPA. Coordinate Quality Improvement Activities (QIA’s), to identify performance area for improvement. Assessments and treatments for variety of medical conditions. Ensured efficacy of treatments through monitoring of treatment regimes. All Required functions applicable as stated above with other case management positions. Management of Hospice patients, and bridging to hospice care.
