Daily Duties at A To Z Health Care :

Primary Case Management of Patients. Responsible for primary care/case management of patients. Delegation of staff. Assessments and treatments for variety of medical conditions. Ensured efficacy of treatments through monitoring of treatment regimes. Prioritizing of treatment and maintain patient flow. Including but not limited to on call. Provide treatment with in scope of practice as defined by the state of law. Coordination of care with patients, families, physicians and health care team. Develop and implement of the patient’s plan of care. Ensure HIPPA compliance and Regulations of MCR and MCD is maintained at all times. Treatment of various chronic diagnoses in the adult patient. Provide care, ensured safety and well being of all patients. Managed case load of 20 + clients, including treatments of IV therapy, TPN, wound care, patient education. Medication Management. Provide behavioral/emotional support.
