Daily Duties at Gold Coast Home Health:
Management of patients in my assigned area for coordination of care, treatments and follow up per physician orders and directions. Responsible for communication with physicians with concerns and documentation of treatments and communication with team members and physicians. Worked in conjunction with home care aides and physical therapists to ensure optimal care. Flexible in case assignment and area worked. I would take patients in areas that were not within the normal operating area for the agency. Effective in teaching patients about their disease process, medications and wellness. I was able to teach a post open heart private pay patient the basics he needed to know in post operating problems, diet, medications and follow-up in one visit.
