Daily Duties at Skyridge Medical Center:
Primary responsibilities include risk assessments for patients on mental health holds, or with altered mental status, depression or substance abuse withdrawal. Performed Social Service consults for suspicion of neglect, abuse, abandonment and help with referral to community resources. Offered and helped with the process for palliative care, hospice care and grief counseling. Secondary duties involved assistance with discharge planning to skilled nursing facilities, rehabilitation, home health care, wound care, home antibiotics, dialysis, and nutrition. I provided clinical review to patients’ insurance carriers. I worked effectively with a diverse multidisciplinary team including physicians, nurses, PT/OT, dietary, pharmacy and religious counselors.
