Daily Duties at St. Anthony North Hospital:
To assist higher risk patients with a safe discharge plan because of lack of insurance coverage, non-compliant with medical care or patients need community resources. I must assess patient’s ability to safely maintain within the community after discharge, whether that is medically, mentally or socially. I also serve in the role of case manager setting up discharges to skilled nursing, long term acute care, home health, durable medical equipment or hospice. I must submit and follow guidelines for PASRR, ULTC100, and all insurance carriers. I must maintain current, accurate and respectful communication with all disciplines within the hospital including but not exclusive to doctors, nurses, physical therapist, occupational therapist, respiratory therapists and chaplains.
