Assess the needs of patients in order to develop an education plan that would best meet their needs and establish appropriate goals for them. Provide education for patients and their support persons on self-management of their diabetes. Assist colleagues in acquiring knowledge and skills in insulin pump therapy and continuous glucose monitoring. Participate in the development of and the teaching of general diabetes education classes. Participate in the development of diabetes support groups Participate in the planning of and implementation of programs for staff development and education of Primary Care Physicians and their staff
