Daily Duties at Cheer:

Connecting disabled individuals and seniors with the Medical services and service required needs of daily living. CHEER offers many services: Meals on Wheel, C.N.A., Care Management, Home Services, Housekeeping, Support Groups, Care Giver Resources, Social Activities, Fitness Centers, Mobile Mini Mart, Transportation, and Volunteer Programs.Case Manager and Assessment Nurse for a nonprofit organization in Sussex County, Connecting disabled individuals and seniors with the Medical services and service required needs of daily living.Home Health Registered Nurse/ Case Manager......Up at 7am, to be at the office by 8am. Receive my assignment for the day. Gather any supplies I will need for the day, order any supplies that need to be delivered to the patients home. Pre write any lab work requisitions that I need for the day. Call the patients and plan the time I will be there. Usuall see 3 in am, 3 in afternoon. Usually in the home 40-50 minutes ( no less than 30 minute visits allowed), which I could never do because you can teach teach teach until the cows come home! Finish my day obtaining all labs results, calling the doc's about their patients if needed and and new orders.A case manager serves as an advocate for his clients. I am responsible for making an in depth assessment of a client's needs. I will then refers them to resources that will meet or satisfy those needs.Within a social services environment, a case manager prioritizes cases once they have been assigned to her by her supervisor. She immediately reaches out to clients to assess their most urgent needs. She keeps thorough records of each of her cases, including every call, home visit and referral. She develops a detailed plan of action to meet the needs of her clients. She works with other agencies to keep abreast of services and resources. She maintains the confidentiality of her clients. She keeps in contact with clients once their situation has been resolved.Within a medical environment, a case manager performs an in-depth mental or physical health analysis of her patient. She offers counseling for patients both individually and in group settings. She networks with external agencies to provide support services to her patience. She keeps detailed records of her patients' progress within the process. She creates an action place to treat patients. She maintains the confidentiality of her patients.act as a patient advocate; investigates and reports adverse occurrence, and perform staff education related to resource utilization, discharge planning and psychosocial aspects of healthcare. promote effective and efficient utilization of clincal resources. collaborate with clincial staff in development oand execution of the plan of care, and achievement of goals. I direct and participates in the development and implementation of patient care policies and protocols in order to provide advice and guidance in handling special cases or patients need. coordinate the provision of social service to patient, families and signifivant other to enavle te to deal with the impact of illness on individual family functioning and to achieve maximum benefits and healthcare service.Obtaining the history of the injury or illness Reviewing/evaluating the treatment plan Obtaining medical history of the claimant Review of employment history, hobbies and interests Evaluate the financial status, ability to perform activities of daily living (ADL) and� family support systems of the injured individualA claimant profile which includes diagnosis, prognosis and outline of desired treatment Identification and scheduling with appropriate medical specialists Evaluation of the treatment plan Schedule appointments for implementation/follow up of treatment plan. Identification/Evaluation of prescribed medications and therapy Description of any special needs involving medical equipment and assistance for their procurement through rental or purchase and delivery Evaluation of any Psycho-social, ADL, financial and other needs for the individual with appropriate follow up.Case management also plays an enormous part in geriatric care and involves helping patients and families find access to community resources and make long term plans for care
