Daily Duties at South Central Los Angeles Regional Center:
Provide case management services for medically fragile and chronically ill infants and children, and developmentally disabled school aged children and adults; including judicially involved adults on diversion and persons diagnosed with a mental illness. Develop individual program plans for each client in conjunction with family members, local education agencies, health care agencies, mental health professionals, and publicly funded entities mandated to provide services; drafting subsequent reports. Coordinate all services as determined appropriate; maintain ongoing case notes. Coordinate and participate in Individual Education Plan (IEP) meetings with local education agency; work with/report to court regarding family reunification plans; conduct psychosocial and family needs assessments, behavioral assessments, and make subsequent referrals. Participate in assessment of developmental needs of children; develop program plan. Complete initial intake interviews, transition planning. Establish and maintain inter-agency collaborations. Senior Coordinator: schedule and chair interdisciplinary team meetings, coordinate and monitor initial out of home placements; assist in research projects and development of needs assessments; develop and implement staff training, assess new staff performance, training needs; provide evaluation of staff projects.
