Daily Duties at New York City Health & Hospitals Corporation Office Of Correctional Health Services:

• Assigned to the Eric M. Taylor Center EMTC/CIFM on Rikers Island, (EMTC is one of 15 facilities within New York City's large jail system). • Duties included but were not limited to: identifying inmates who were considered to be seriously and persistently mentally ill (SPMI) and who were in need of continued mental health services upon discharge back to their communities. • Screening inmates for program eligibility, completing a comprehensive intake assessment, developing a comprehensive discharge plan, working in cooperation with the NYC Department of Correction and various criminal justice agencies, collecting all mental health documentation and completed any necessary applications (i.e. NYC HRA 1995 Housing Application). • Ran a weekly discharge-planning group to assist inmates in preparing for their release. • Establish and maintained a working relationship with various mental health providers in the 5 boroughs of New York City.
