• Provided drug information to medical team when required. • Made drug recommendations to medical team during morning rounds. • Performed Drug Utilization Review (DUR) on patient profiles and made the necessary changes to drug regimen based on clinical situation, e.g. adjusting the dose of a drug in a patient with renal failure. • Gave presentation to medical team on management and treatment of Ascites. • Attended Grand Rounds once a week on various disease states. • Provided detailed consultations to patients regarding appropriate medication administration and usage. • Helped determine appropriate outpatient medications at time of discharge. • Involved in monitoring patient’s labs e.g. renal function, electrolyte imbalances, and making the necessary recommendations. • Assisted physician in HIV clinic with patient’s medication regimen.
