Daily Duties at University Hospital:

• Responsible for the complete and accurate order entry of medications onto patients profile using Meditech database. • Verified the accuracy of all medications being dispensed. • Provided drug information to physicians and nurses. • Monitored APTT/INR values on select anti-coagulants e.g. Heparin, Argatroban, Refludan, and Coumadin and made appropriate dose adjustments per protocol. • Counseled patients on Coumadin therapy as per physicians medication order. • Contacted physicians in order to clarify medication orders. • Ensured that Anti-coagulant and/or Anti-biotic therapy were appropriately administered and discontinued according to Surgical Care Improvement Project (SCIP) protocol. • Adjusted the dose of drugs based on patients estimated creatinine clearance and other clinically relevant lab values. • Converted patients from IV to PO per protocol.
