Daily Duties at Metro Health Hospital:

Provides Pharmacy Services at a proficient level of Pharmacy practice. Upholds the standards of the system wide customer service program. Provided said services for third shift operation of InPatient Pharmacy. I was hired and trained at MetroHealth to be a third shift Pharmacist. I was there to run the Pharmacy Department form 9pm to 7am, 7 days on, 7 days off. It was understood that we were responsible for the department, and management did not want any phone calls from us. There were always 2 Pharmacists on 3rd shift. That was a great help; if one Pharmacist didn't know something, the other probably did. We had 3 technicians on shift also, and most nights we stayed busy. The 1st order of the shift was checking the Cart Fill. There are 700 beds in this hospital, and every patient had a drawer for their meds. We checked this work to ensure it was correct. It would take 2 Pharmacists 60-90 minutes to complete this check. After that, we would get a report from the 2nd shift Pharmacist about any ongoing issues. We would take over from there, and run the show. A smooth night entailed having orders faxed down to us from the bank of faxes sitting behind us. There were 5-6 faxes that printed orders through the night. We would input the orders into a computer, print labels that our technicians would fill and bag with the labels. Every hour we would check their work, sign off on it, alnd send it to the floors through a tube system. This process would repeat hourly through the night. Around 5am, we would run a fill program for the daily IV's chick would print labels for the comming day. At 7am, we were relieved btythe 1st shift Pharmacist, tell them what was up, go home, sleep and repeat x7days. But most days weren't that calm. In a big hospital like MetroHealth, there were always problems, and they often appeared at night. When problems came, they had to be accessed. Can it wait?, If not, who can handle it or who can be spared? If we were lucky, we could assign a technician to handle it. Loosing1/3 of your technician help was better than 1/2 your Pharmacist help. Once manpower was assigned to a problem, workflow would often suffer. When the problem was solved, we would have to work fast and hard to try to catch up the backlog of orders. I will list and explain some examples of problems. Printers would break, jam, or quit. This was especially problematic because it always took a while to discover(they were in another room). Doctors would order meds we didn't have or could not get. It was often hard to communicate effectively with the doctors, especially at night. Some orders had to be produced by hand. This was very time-consumming. The formula had to be found, the ingredients needed to be weighted, and then mixed together. This usually occurred at only the worst times. Computers would lock or crash- this would hobble the pharmacy. Orders could be filled by hand, but it took extra time and effort. And then the orders still had to be entered into the computer once it was back online. The tube system would go out- this happened way too often. This would require us to walk the meds to the floors. This ate up time and manpower and always put us behind. Staff would call off to us for the following 1st shift. This opened up a new can of worms that a Pharmacist had to handle. It wasn't nice and was often contentious, and our technicians had to make up the short-comming. This is a prtial list- its been 6 years since I last worked there. It was demanding and challenging job that I enjoyed for those reasons.
