Daily Duties at Gateway Hospice:
See and manage caseload of patients,Oversee and manage LPNs and CNAs.Make up schedule to provide visit coverage.Monitor patients for any changes,increase in pain,decreasein appetite ,weight,wounds .Family dynamics.work with pts famalies,facility staff,pharmacy,Admissions,transfers to SNF form home for respite stay.On Call.Work closely with IDT to keep current with changes to patients
