Conducted assessments to ensure that mental health and other needs are identified Through a United Way program: facilitated empowerment, life skills, job readiness and health/wellness groups for clients who are chronically or long-term homeless Implemented solution-focused, cognitive behavioral techniques and motivation to engage clients in their own problem solving processes Assisted clients with prioritizing goals and creating long and short term planning as they work to achieve self sufficiency and improve their state of well-being Represented AWDA through service on a Care Coordination Group approved by the Regional Commission on Homelessness in an effort to conduct joint problem-solving, resource sharing, and creating care management plans for homeless persons with disabling mental health and co-occurring conditions Counseled evacuees of Hurricane Katrina
