Daily Duties at Sw Washington Medical Center:
Hospital Acute Care Discharge Planning, Case Management, Utilization Management and chart review to ensure accurate insurance billing and reimbursement and appropriate level of care for patients. Performed chart reviews to determine inpatient versus observation patient assignment. Coordinated with insurance providers to determine appropriate level of care for patients. Performed Concurrent Reviews. Coordinated discharges for patients to SNF, ALF, AFH and Home Health. Arranged for appropriate DME for patients as needed. Collaborated with physicians, staff and community healthcare providers to develop a safe discharge plan for patients.
