Entered physician orders in the pharmacy computer system for four large long-term care facilities. Assessed regimens for appropriate medication usage, allergies, drug interactions, and dosing. Provided drug information to nursing staff. For the hospital, performed clinical drug monitoring including recommending IV to PO substitutions for appropriate patients and monitoring for vancomycin overuse.
