Daily Duties at Eisenhower Medical Center:
Provide coordination of care in multiple service areas utilizing the right resources, at the right time and the right level of care in the episodic acute setting. Utilizing a collaborative process assess, plan, implement and evaluate the options and services required to meet an individual’s health needs during an acute encounter. Communicate with third party payers to authorize service, and ensure quality, and effective outcomes. Perform concurrent review of the medical records.
