•Responsible for the overall care management of a diverse caseload of 70 consumers. •Responsible for ongoing care planning, service arrangement, follow-up, and reassessment. •An advocate as well as the gatekeeper of service provision for the consumer. •Develop and implemented patient management plans, recorded progress notes, and assisted in provision of continuity of care •Demonstrate track record of delivering quality, cost effective home based community services through competencies in problem solving, decision making, and quality assurance. •Serve as a senior member on team while contributing to staff development within the department by serving as a mentor to incoming care managers. •Implement objectives, polices, and processes of home based community services to the elderly.
