Daily Duties at Mercy Hospital:

• Conducted reviews and assessments of patient data to determine need for medical social work intervention. • Conducted assessments of patients’ medical and social data in order to identify problems to be addressed and developed appropriate psychosocial intervention strategies. • Participated as a member of the treatment team by providing input to the development of modification of treatment plans by communicating to health care team members’ social, emotional, and environmental problems which may affect patient response to treatment. • Provided problem specific and time limited counseling to patients and families directed at supporting and maximizing the social functioning and adjustment of the patient. • Coordinated discharge planning process for patients, assisted in arranging concrete services such as transportation, post-hospital planning, housing, legal services, financial assistance, vocational counseling and/or make necessary referrals for community services. • Coordinated discharge planning process especially for Renal Patients in terms of arranging for transportation services to and from the ambulatory Renal Program Nursing Home Placements, psychosocial assessment, and other services. • Assisted in the evaluation/disposition of medical patients with mental health problems, including psychosocial evaluations and transfers to psychiatric facilities. • Acted as a liaison between the department and various community agencies providing social services. • Maintained and enhanced professional skills through participation in appropriate educational and professional activities.
