Daily Duties at Saint John Detroit Riverview Hospital(Closed):
• Conduct reviews and assessments of patients data to determine need for medial-social work intervention • Complete psychosocial evaluations in order to develop the appropriate discharge plan/psycho-social interaction strategies for identified problems. • Participate in the weekly multidisciplinary rounds to discuss patients’ care in order to establish a treatment plan. • Coordinate discharge planning process for patients including transportation, nursing home, adult foster care, psychiatric placements, financial assistance, visiting nurses, outpatient infusion, IV antibiotic orders, durable medical equipment, legal services, vocational counseling and/or arranging necessary referrals for community services. • Maintain and enhance professional skills through participations in appropriate educations and professional activities.
