• Complete the Clinical review of medical charts for utilization review purposes for insurance companies. • Conduct interviews and psychosocial evaluations for inpatients • Arrange for skilled/basic nursing home, acute/subacute rehab, adult foster care placements. • Complete arrangements for outpatient hemodialysis patients. • Provide referrals for community resources such as substance abuse and outpatient community mental health facilities • Prepare letters for The Probate Courts legal guardianship on behalf of the hospital/families for patients. • Arrange for homecare, infusion, and durable medical equipment.
