Representative and Case Manager responsible for educating Physicians, Referral Coordinators, Case Managers, Wound Care Clinics, and other Professional Contacts on the range of services such as Skilled Nursing and Physical Therapy for patients in a home environment. Liaison and educator with Medicare Home Health Benefit. Maintain case load communication and evaluation between clinicians and patients throughout care certification period.
