Daily Duties at Van Dyk Healthcare :
Worked with a team of medical professionals to deliver the highest of quality of healthcare services, from admission to discharge. Responsible for all discharge planning for patients (and family). Conducted weekly multidisciplinary team meetings to review care plans and MDS (minimum data set). Engage the patients at all points to develop and evaluate plan of care. Provide support groups for families on a variety of topics relevant to their loved ones.
