24 hour case management of home bound patients as requested by their physician. Assessed patient needs for nursing and/or therapy with patient and family input. Performed appropriate and necessary nursing skills promoting the healing process. Began discharge planning at admission to promote a smooth transition back into the community with or without needed support.
