Daily Duties at Westborough State Hospital:

Provide safety and containment to unit. Consistently monitored milieu and activities and mental status of clients to be pro-active and intervene prior to the escalation of a client. Also monitored milieu for contraband or any item that a client could potentially use to harm self or others. Planned a structured evening with different activities, to keep patients busy and content to prevent restraints. When a client showed signs of escalation, utilized staff members with strongest therapeutic relationship with client to de-escalate client. Although the quiet room often needed to be used, attempted to use least restrictive interventions, such as a brief time-out for client to regain composure. To the best of my ability, I had other clients engaged in an activity when a client required restraints. to prevent a snowball effect. Attempted to utilize chemical restraints prior to mechanical restraints, due to the restraint reduction effort by the department of mental health. Accurately transcribed orders, was a team player, planned care according to best practice. Worked to establish therapeutic relationships with clients, but many were too ill, truly engage in working relationships. Learned that each clients' mental status could change unpredictably. Did not expect mental health workers to take full responsibility for checks and monitoring of quiet room. Often relieved Mentl Health Workers from quietroom so they could take breaks, or take clients off unit. If we had several restraints, I often monitored the quiet room because policies required frequent checks by an RN, and I would work on the restraint forms while monitoring the quiet room. I was usually not available for long periods of time, until clients were in their rooms, but clients began to settle for bed starting at 8P. Westborough was a challenging hospital to work at, because the majority of clients had little or no motivation to change their behaviors, and they did not have a goal of discharge. They had become disillusioned by their past failures in other placements, and except for a few clients who truly worked towards discharge because they had family support or an alternative support system they held no true goal of discharge. I delivered optimal care based on evidence-based practice, the setting was simply of high acuity.
