Daily Duties at Synergy :

Carried a case load of medical-surgical clients teaching and monitoring their health status and mental status. Followed clients through admission to discharge. Taught safety issues, medication regimens, provided medication planners which I either filled or monitored for accuracy. Developed plan of care, and sent to M.D. for approval and M.D. orders. Initiated the psychiatric Home Health Component for the agency which required marketing the program to referral sources. Developed therapeutic working relationships with my clients by developing trust. Maintained professional boundaries. Taught clients their medication regimens often utilizing visual aids. Some of my psychiatric clients needed to have their medications stored in locked boxes due to suicide risk. Worked with clients to set measurable goals, and to learn adaptive coping skills. Treated all clients with respect and dignity. Monitored vital signs at every visit. Accurately and timely documented care provided. Always willing to assist the agency with unexpected admissions or re-admissions. Met all deadlines
