Daily Duties at Hph Hospice:
Provides information to the patient and family regarding the hospice philosophy of care. Assesses and identifies patient’s physical needs and incorporating psychosocial, spiritual, and cultural needs. Supervised nursing staff regarding intravenous skills, pain medication titration, symptom management and patient/family relationships. Monitors patient’s medications and supplies. Educates patient and family regarding symptom control, palliative care and patient care skills. Provides emotional support to patient and family. Assist staff with documenting care given in a timely manner and on appropriate hospice forms. Participates in interdisciplinary team meetings; reports changes in the patient’s condition and resolution of problems relating to nursing care; and refers relevant patient and family needs to Support Services. Updates IDT Plan of Care regarding problems and interventions. Schedule LPN and hospice aide for oncoming shift. Mentor LPN and hospice aide through demonstration of skills, providing feedback on job performance and coordinating activities to enhance job performance. Apprise Clinical Manager of changes in patient’s condition, care needs and concerns of patient, family and other health care providers. Participate in orienting new staff, students, volunteers and board members. Participate on committees and projects, as assigned. Act as a resource to peers, orientees and other staff members. Participate in activities and professional development that enhance hospice nursing skills. Participate in the education process of the Company. Exemplify behaviors supportive of internal mission statement.
