Daily Duties at Pharmacon Consulting Corp:
Provided pharmaceutical care services to approximately one thousand nursing home residents and sub-acute units monthly.Core activities: Review drug use. Perform physical assessment concordant with the disease and drug therapy.Appraise and advise regarding adjustment of medication regimen. Cost minimiztion by reducing the number of medications, eliminating therapeutic duplications, and recommending therapeutic substitution. Identify and resolve medication related problems, medication errors.Evaluate,identify and address potential risk for adverse event secondary to chronic drug use. Apply consensus reports gidelines to provide recommendations for designing appropriate careplans, therapeutic goals, intergtrations of nondrug therapy to achieve wellness and for ameliorations of disease progression. Document professional services, cost reduction. provide drug information, education to augment the effectiveness of medical providers. Engaged the nursing aides(CNA) in patient care plans and drug regimen review. Review CNAs daily reports for omissions and irregularities. Compared their reports to the nurses progress notes and care plans to rule out conflicts and risk for medications problems. Evaluate literature for decision-making process. Make recommendations for therapeutic interventions. Contribute to plan development for prevention, treatment and monitoring based on pathophysiology, clinical presentation and resident specific characteristics.Perform nursing unit inspections. Adresss and correct irregularities. Report findings and resolutions to director of nursing.
